Welcome to Aspire Health Services Inc ("Company," "we," "our," "us"). These Terms of Service ("Terms") govern your access to and use of our services, including in-person treatments, virtual consultations, website usage, and related communications in connection with our advanced wound care services (collectively, the "Services").

By accessing or using our Services, you agree to be bound by these Terms and our Privacy Policy. If you do not agree with any part of these Terms, please do not use our Services.

1. Eligibility
You must be at least 18 years of age or have a valid legal guardian's consent to use our Services. By using our Services, you represent and warrant that you meet all eligibility requirements outlined in these Terms.

2. Medical Disclaimer
All medical treatments are subject to evaluation by a licensed healthcare provider.
Our Services do not substitute emergency medical care. In case of emergency, call 911.
Treatment results may vary depending on individual health circumstances and adherence to post-care instructions.
We reserve the right to refuse or terminate care if it is outside the scope of our clinical expertise or for patient safety.
3. Services Offered
We provide advanced wound care, including but not limited to:

Negative Pressure Wound Therapy
Debridement Procedures
Bioengineered Tissue Applications
Patient Education and Follow-Up
Home-Based or Clinic Wound Assessments
Telehealth Consultations (if available)
All services are administered by licensed medical professionals in accordance with federal and state regulations.

4. Appointments, Cancellations, and Payment
Appointments: Require advance scheduling. Late arrivals may forfeit appointment time.
Cancellations: Require at least 24 hours’ notice. Late cancellations or no-shows may result in fees.
Payments: Patients are responsible for payment of services not covered by insurance. Co-pays, deductibles, or full payment may be collected at the time of service.
We accept most major insurers. Insurance verification is not a guarantee of coverage.

5. Telehealth Services
If offered, telehealth consultations comply with HIPAA and applicable telehealth laws. Patients must have secure internet and a compatible device. Telehealth may not be suitable for all wound types or conditions.

6. Privacy and Confidentiality
We are committed to protecting your health information in accordance with HIPAA. Please refer to our separate Privacy Policy for details on our information practices.

7. Patient Responsibilities
You agree to:

Provide accurate, complete, and updated medical information.
Follow treatment plans and provide informed consent.
Pay for services as agreed upon.
Treat staff and providers with respect and courtesy.
8. Limitation of Liability
To the fullest extent permitted by law, Aspire Health Services Inc and its staff shall not be liable for any indirect, incidental, or consequential damages resulting from your use of our Services. Services are provided "as is" and "as available."

9. Termination
We reserve the right to terminate or suspend care and access to the Services at our discretion, including for violations of these Terms or inappropriate conduct.

10. Governing Law
These Terms are governed and construed in accordance with the laws of the state in which we operate. Any disputes shall be resolved in the courts of that jurisdiction.

11. Changes to Terms
We may update these Terms from time to time. We will notify users of material changes, and continued use of our Services constitutes acceptance of those changes.
